Here is a short list of some FAQs which should answer some of your questions.

One form of primary I.D. and one form of secondary I.D. (Most common forms are Government birth certificate with raised seal , and social security card ) a full list of primary and secondary I.D. can be found in the state drivers manual and a blue card issued by Cannon River Driving School.
You have to be 15 yrs old, and have obtained a blue card from Cannon River Driving School and have the proper I.D.
You have to be 16 yrs old, and had your permit for at least six months, and have a white card from Cannon River Driving School.
Your permit, your white card from Cannon River Driving School, and have proof of insurance in the car, for the car you are using on the test.
